Summary

The selected document does not have an index card template attached to it.

Workaround

Assign an index card template to the affected document by performing the following steps:

1. Select the affected document.
2. From the Tools menu, select Change Doc Template. The Select New Template dialog box will appear.
3. Under New Template, select the index card template that you would like to assign to the document.
4. Click OK.
